I am voluntarily taking part in this program and releasing LivBTR™ Wellness Solutions and its coach(es)/instructor(s) of any liability.  In acknowledging this waiver and taking part in the Gut Reset program I am agreeing to take full responsibility for any current limitation or health risks.  I understand there may be adverse health consequences if I surpass my ability or the health limitations my physician has recommended.  I agree to self-limit my exertion through good judgment and stop any processes that I feel exceed my capabilities.

This program was created through our health backgrounds, training, skills, and life experiences as well as practical application with our clients.  We support our clients—spiritually, mentally, emotionally, and physically.  We are not medical doctors, dieticians, or nutritionists.  We specialize in hormone assessments, nutrition for specialized populations, health, and life coaching.  We make no claims to any specialized medical training, nor do we dispense medical advice or prescriptions.  Your program is not intended to diagnose or treat any diseases.  It is intended to be provided for informational, educational, and self-empowerment purposes ONLY. 

Please consult with your doctor or health care practitioner if you have any questions regarding this program and your specific health history.  If you’re working with a LivBTR™ coach or instructor please ensure your information is up to date.  It is your responsibility to make your own well-informed decisions based on what is best for your unique genetics, culture, conditions, and stage of life.

In agreeing to this form, I agree to participate in LivBTR™ Gut Reset Program at my own risk.  I understand my coach/instructor will supply me with as much knowledge and information as he/she can to ensure safe guidance through the course.  

I hereby release LivBTR™ Wellness Solutions and my coach/instructor from any liability, injury, or property damage/loss sustained due to, or as a result of my participation in this program. 

PAYMENT

After you register for the program your coach/instructor will begin working on your program (if you purchased a program that requires tests, meal planning, workouts, etc.)   You can cancel access to the Gut Reset Program up to one day before the program is intended to begin provided no additional programs or packages have been added.  For example, a meal plan add-on requires 7 days' notice for cancellation as preparation starts on your plan before the plan commences.

PRODUCT DELIVERY

If you purchased a program that includes products, the products will be shipped within 24 to 48 hours of the purchase of the Gut Reset program.  Delays may occur receiving products based on demand, ship-to location, and shipping limitations.  It is best to ensure you sign up for a program 7 to 10 BUSINESS DAYS before the program start date to provide time to deliver any necessary products.

DIAGNOSTIC TESTING

The cost of shipping to the testing facility is at your expense.  Cost to ship depending on the place of residents.

Buyer is responsible for ensuring they follow instructions when shipping samples to avoid additional costs.  Instructors are typically sent by email.

Programs are non-refundable once a testing kit has been shipped to the buyer. 

Once your month has begun there will be no refunds on programs or products that can be downloaded, viewed, copied, or stored in an electronic format.

PROTOCOL

In our program communication is necessary.  You will get back only what you put in.  We expect weekly check-ins, communication with any issues with your program as well as with any other pertinent concerns.  We want to ensure we support you in any way you need. This is a partnership and we must work together to uncover your greatest potential.

The Gut Reset Program is guided by Brenley Cunningham, Holistic Health Practitioner, who specializes in gut and hormone health, and diagnostic testing.

We appreciate unexpected circumstances in life can impact staying on track.  While this program encourages greater success following the 30-day process, we allow access for 10 additional days following the last day of the reset if you need to catch up.
